Shape Up Duke Punch Card to Duke Fitness Club

Get Fit for Free. Participants receive a card for five free visits to a (participating) local Duke Fitness Club member gym of their choice. Exercise is good medicine. Working out is something you do for yourself that helps you feel better physically and mentally.

To participate, you must be registered in Shape Up Duke.

Steps to Take:

  1. Download a waiver form. (Click to download)
  2. Mail/fax or bring a copy of your waiver form to LIVE FOR LIFE® who will provide you with a Shape Up Duke Punch Card.
  3. Visit gym and present Duke Punch Card at registration desk.

Nutrition Consults - Consult one-on-one (in person or by phone) with a Registered Dietitian for ways to improve your health through your diet.  Call 684-3136 (Option 1) or e–mail lflprogm@mc.duke.edu to schedule a consult. To maximize your time with the dietitian, complete 3 days of a food journal to bring to your appointment.

Mall Miles In The Area: If you can step inside a mall without getting sidetracked with shopping, they are great places to walk, especially during inclement weather.
